Appender should ignore messages of 'cause' exceptions

      Description

      The existing appender determines whether to create a new issue or append to an existing issue based on the hash of the stacktrace that relates to a to-be-logged exception.

      The first line of the stacktrace is not included when calculating the hash. This line is skipped, as it contains the exception message. The exception message is often slightly different for invocations of the same exception (as it typically includes data specific to that one instance, such as usernames, or string representations of offending values.

      An exception can have a linked 'cause' exception. The appender should be improved in such a way that when calculating of the hash, the message that is included in such a 'cause' exception is also ignored.
