RFC 6265 http://tools.ietf.org/html/rfc6265 specifies how a servers might maintain
a stateful session for http requests using set-cookie and cookie headers.
As you can see, once the server sends a cookie to the client, it will send the cookie
in each subsequent requests including the http request that initiates the connection.
RFC 6455 (see section 1.3) support RFC 6265 cookies in the handshake request.
apart from URL rewriting.
So, the association is not exactly done by our specification. What we do in our specification
is to give APIs for application to make use of this association.
